DESCRIPTION An anal fissure is a small split or tear in the anal mucosa that may cause painful bowel movements and bleeding. There may be blood on the outside of the stool or on the toilet tissue following a bowel movement.

An anal fissure is an ulcer in the anal canal. It can vary from a small split in the lining of the anus to a deep ulcer down to the anal sphincters. There may be a small skin tag associated with it and this is called a 'sentinel pile'. It is nothing to do with hemorrhoids.
Anal fissures are extremely common in young infants but may occur at any age. Studies suggest 80% of infants will have had an anal fissure by age one. Most fissures heal on their own and do not require treatment, aside from good diaper hygeine. However, some fissures may require medical treatment.
The incidence of anal fissures decreases rapidly with age. Fissures are much less common among school-aged children than among infants.
In adults, fissures may be caused by constipation, particularly when passing large, hard stools, or by prolonged diarrhea. In older adults, anal fissures may be caused by decreased blood flow to the area.
Anal fissures are also common in women after childbirth and people with Crohn's disease.
A hard, dry bowel movement can cause a tear in the anal lining, resulting in a fissure. Other causes of a fissure include diarrhea and inflammation of the anorectal area.
Anal fissures may be acute (recent onset) or chronic (present for a long time or recurring frequently). An acute fissure is usually due to altered bowel habits while a chronic fissure may be either due to poor bowel habits, overly tight or spastic anal sphincter muscles, scarring or an underlying medical problem.

Some fissures are asymptomatic (ie cause no symptoms), but most patients experience severe pain on opening the bowels, which can last up to an hour afterwards. There may be associated bleeding, which is usually bright red and is on the stool or on the toilet paper.
