Aphthous stomatitis

DESCRIPTION Aphthous stomatitis is an illness that causes small ulcers to appear in the mouth, usually inside the lips, on the cheeks, or on the tongue.

The exact cause of this disease is not known. There are many factors that are thought to be involved with the development of canker sores, including:

Aphthous stomatitis is usually seen in children and adolescents from the ages of 10 to 19 years. For about one-third of the children affected, lesions continue to reappear for years after the initial outbreak.

These ulcers are not contagious and cannot be spread from one child to another.
Recurrent aphthous ulcer ( RAU) is classically divided into 3 clinical forms:
  • RAU minor - RAU minor is the most common form, accounting for 80% of all RAUs. Discrete, painful, shallow, recurrent ulcers measuring from 3 mm to smaller than 1 cm in diameter characterize this form. At any time, 1-5 ulcers can be present. RAU minor occurs on the labial and buccal mucosa and on the floor of the mouth. Lesions heal without scarring within 7-10 days.
  • RAU major - RAU is formerly known as periadenitis mucosa necrotica recurrens. This form is less common and is characterized by oval-shaped ulcers that are 1-3 cm in diameter. In this more severe form, 1-10 major aphthae may be present simultaneously. Ulcers are larger and deeper, and they may coalesce and often have a raised irregular border. Upon healing, which may take as long as 6 weeks, the ulcers leave extensive scarring, and severe distortion of oral and pharyngeal mucosa may occur. This form affects the lips, the soft palate, and the fauces more commonly than other sites.
  • Herpetiform RAU - This least common form has the smallest of the aphthae, measuring 1-3 mm in diameter. The aphthae tend to occur in clusters that may consist of tens or hundreds of minute ulcers. Clusters may be small and localized, or they may be distributed throughout the soft mucosa of the oral cavity.

SYMPTOMS The following are the most common symptoms of aphthous stomatitis. However, each patient may experience symptoms differently. Symptoms may include:
  • Ulcers in the mouth, usually inside the lips, on the cheeks, or on the tongue
  • Ulcers that are covered with a yellow layer and have a red base
  • No fever present (in most cases)
  • Lesions usually heal in 7 to 14 days
  • Lesion tend to recur
