A diverticulum (if there are more than one they are known as "diverticula") is a protrusion of the inner lining of the intestine through the outer muscular coat to form a small pouch with a narrow neck. The commonest site for diverticula to develop is the lower left part of the colon. The presence of diverticula is often referred to as diverticulosis.
Some people are born with a diverticulum, most often from the small intestine. Most diverticula develop during later life and are more and more common with increasing age.
A low-fibre diet is seen as a major cause of diverticular disease because it often leads to constipation, which in turn can cause pouches to form in the weak spots of the colon wall due to the excess pressure required to force small, hard and dry stools through the bowel. Stress, smoking, obesity, coronary heart disease, gallbladder disease and a family predisposition towards the disorder are also believed to increase one's chances of developing diverticulosis/diverticulitis later in life.
Diverticular disease may occur in several forms.
  • Diverticula of colon: Herniation of the colon mucosa through the muscular layer, usually at the site of a perforating artery, lying between two layers of serosa in the mesentery. They are more common in the sigmoid and distal colon and increase in numbers with age.
  • Diverticulitis: An abscess or prediverticular inflammation initiated by the rupture of a mucosal microscopic abscess into the mesentery. Such infection may progress, fistulize into the genitourinary system, obstruct, or spontaneously resolve. Develops in about 5% of subjects with diverticulosis each year. Over a lifetime about half of patients with diverticulosis develop inflammation.

    • Only 10-25% of subjects harboring diverticula will develop symptoms (possibly related to coexistent irritable bowel disease)
    • Pain - due to tension in colonic wall, mostly in left lower quadrant, worse after eating, some relief following bowel movement or passage of flatus
    • Diarrhea or constipation
    • Palpable mass in left iliac fossa - firm, tender
    • Abdomen may be distended and tympanitic
    • Absent signs of peritoneal inflammation
    • Melena, hematochezia if diverticula bleed
  • Diverticulitis
    • Pain - acute onset, mostly localized in left lower quadrant. Prominently associated with tenderness in same region.
    • Fever with chills as severity increases
    • Anorexia, nausea, vomiting
    • Constipation or diarrhea
    • Rebound tenderness, involuntary guarding, board-like rigidity
    • Palpable mass - tender, firm, fixed
    • Abdomen distended and tympanitic
    • Bowel sounds depressed or could be exaggerated if obstruction ensues
    • Dysuria, frequency if bladder involved
    • Pneumaturia, fecaluria if colovesical fistula develops
    • Rectal exam may reveal tenderness, induration, mass in the cul-de-sac
    • Enterocutaneous, enterovaginal, and perirectal fistulae may be initial manifestation
