Esophagitis is frequently caused by backflow of acid-containing fluid from the stomach to the esophagus (gastroesophageal reflux). It may also be caused by excessive vomitng, surgery, and medications such as aspirin, ibuprofen, potassium, alendronate, and doxycycline. |
In patients with weakened immune systems caused by HIV and certain medications (such as corticosteroids), esophagitis may be caused by an infection of the esophagus. Esophageal infection may be caused by viruses such as herpes or cytomegalovirus, and fungi or yeast (especially Candida infections). |
The infection or irritation may cause the tissues to become inflamed and can occasionally cause ulcers. There may also be difficulty when swallowing, and a burning sensation in the esophagus. |
The two principal types of esophagitis are corrosive esophagitis and reflux esophagitis. |
-
Corrosive esophagitis - It is caused by swallowing of caustic chemicals (acid or lye) accidentally or in a suicide attempt. The severity of the inflammation depends on the type, amount, and concentration of caustic chemical swallowed. Immediately after swallowing such a chemical, there is severe pain and edema in the throat and mouth. Antidotes are of limited value and gastric lavage must be avoided as this may only increase the damage. Treatment consists mainly of reducing pain and providing nursing care until the esophagus heals.
-
Reflux esophagitis or gastroesophageal reflux disease (GERD) - It is a very common condition. The cause is poor functioning of the musculature of the lower esophageal segment, which permits reflux of the stomach's contents. Severity of symptoms does not correlate with histology. Complications include bleeding, stricture, Barrett's metaplasia.
-
Infectious esophagitis - Bacteria rarely cause primary esophageal infection, although the esophagus can be involved secondarily by direct extension from the lung. The two most common forms of infectious esophagitis are caused by Candida and herpes. Other viruses (e.g., CMV, HIV) and fungi can also cause esophagitis; however, this is uncommon and almost invariably associated with immunosuppression.
-
Chronic nonspecific esophagitis - Chronic esophagitis typically follows corrosive and reflux esophagitis.
