Globus hystericus (GH) is also called psychogenic dysphagia. The diagnosis should be made with caution and only after thorough evaluation. Both the pharynx and oesophagus should be evaluated in patients with pharyngeal dysphagia; about one third of patients with distal oesophageal obstruction have symptoms referred to the neck region. |
Globus hystericus is generally associated with sadness, anxiety or stress. |
Globus hystericus is a symptom of some physical disorders such as reflux laryngitis as well as a classic sign of hysterical neurosis, a psychosomatic disorder characterized by a change or loss of physical function (such as blurred vision or paralysis of the legs) that suggests a physical disorder but instead is an expression of a psychological conflict or need. |
The exact mechanism which causes globus hystericus is not known. The sensation may result from frequent swallowing and drying of the throat associated with anxiety or other emotional states. |
Globus is probably a physiological symptom of certain mood states. |
It is not associated with a specific psychiatric disorder or set of stress factors. Certain people may have an inherent predisposition to respond in this way. |
Statistically, the most common cause is gastroesophageal reflux disease. If the patient is a smoker, tumor in the pharynx, hypopharynx, larynx and cervical esophagus must be excluded. This is most easily performed by obtaining an ENT consultation, where a good pharyngeal, hypopharyngeal and laryngeal examination should be performed either by mirror or by fiberoptic exam and that the physician will palpate the oral cavity and the oropharynx. |
Sometimes it is caused by the muscles in the throat contracting due to anxiety or stress. It feels like you cannot swallow anything and trying to makes it worse. This is another example of a symptom, which will improve if you give it no credibility. It is totally harmless and will not cause you to stop breathing, eating or drinking, it is just very unpleasant. |
