There are two types of nocturnal enuresis. Primary nocturnal enuresis is when a child has never developed complete night-time bladder control. Secondary nocturnal enuresis is when a child has accidental wetting after having had bladder control for six or more months. It's often associated with a period of emotional stress such as the birth of a younger sibling, a bereavement or school worries.
Fifteen percent of 5-year-olds and 10 percent of 6-year-olds experience enuresis. As children mature, there is a gradual reduction in the number of children who are wet at a rate of about 15 percent per year.
Children vary in the age at which they are physically ready to have complete control over their bladders. Many children are not ready for toilet training before the age of 3. Enuresis usually does not indicate an emotional or physical problem. It is more than twice as common among boys as girls. Causes may be due to a maturational delay in the development of bladder musculature and its ability to withstand the pressure of a large urine volume. It may also be related to toilet training that occurred too early or was too coercive. It may also be a symptom of temporary regression, a response to a new situation, to parents who are too controlling or critical, or as a problem of adjustment. Physical causes are rare, but may include lower spinal cord lesions, congenital malformations of the genitourinary tract, infections of the urinary tract, or diabetes.
Some of the causes of bed-wetting include the following:
  • Genetic factors (it tends to run in families)
  • Difficulties waking up from sleep
  • Slower than normal development of the central nervous system--this reduces the child's ability to stop the bladder from emptying at night
  • Hormonal factors (not enough antidiuretic hormone--this hormone reduces the amount of urine made by the kidneys)
  • Urinary tract infections
  • Abnormalities in the urethral valves in boys or in the ureter in girls or boys
  • Abnormalities in the spinal cord
  • Inability to hold urine for a long time because of small bladder
Bed-wetting isn't caused by drinking too much before bedtime. It's not a mental or behavior problem. It doesn't happen because the child is too lazy to get out of bed to go to the bathroom. And children do not wet the bed on purpose or to irritate their parents.
Urine accidents (enuresis) are probably caused by many factors, not just one. Many children with night-time urine accidents have an overactive bladder and a small functional bladder capacity. This means that a bladder may have many muscle spasms that cause an urge to empty the bladder often. The bladder is made of muscle and can grow and shrink as urine enters or leaves. The bladder in most cases is a normal size physically. However, it is not able to hold a normal amount of urine because of muscle spasms.
Other possible causes of night-time urine accidents include hormonal factors, dietary factors, the effects of certain medications, timing of daytime fluid intake and emotional factors. Often, night-time accidents are found to run in families.
Some children who have night-time accidents will also have daytime urine accidents. Causes for this may include urinary tract infections, abnormalities of the urinary tract, problems with bathroom habits, or problems with the nerve control of the bladder.

SYMPTOMS Involuntary urination, usually at night, occurs twice per month or more often.
In primary nocturnal enuresis, children have never achieved complete nighttime control. They have always wet the bed at least two times a month.
As a result, these children may suffer significant psychological stress and develop feelings of low self-esteem (including feelings of shame, inferiority, and fear of being discovered by others). These are the result, not the cause, of PNE.
Families of bed-wetters can experience disturbed sleep, turmoil, and a drain on energy and resources.
