Cholestasis of pregnancy is a condition in which the normal flow of bile in the gallbladder is affected by the high amounts of pregnancy hormones. Cholestasis is more common in the last trimester of pregnancy when hormones are at their peak, but usually goes away within a few days after delivery. Cholestasis is sometimes referred to as extrahepatic cholestasis which occurs outside the liver, intrahepatic cholestasis which occurs inside the liver, or obstetric cholestasis. |
Pregnancy hormones affect gallbladder function, resulting in slowing or stopping of the flow of bile. The gallbladder holds bile that is produced in the liver, which is necessary in the breakdown of fats in digestion. When the bile flow is stopped or slowed down, this causes a build up of bile acids in the liver which can spill into the bloodstream. |
Intrahepatic cholestasis of pregnancy is a familial disorder that is common in Scandinavia, Poland and Chile. In the United States it occurs in less than one percent of pregnancies. All patients with this disorder develop itching. Other symptoms that are associated with intrahepatic cholestasis of pregnancy include: jaundice (25%), nausea/vomiting (5-75%), and abdominal pain (9-24%). Liver function test abnormalities are common as well. |

The classic symptom of intrahepatic cholestasis of pregnancy is intense generalized pruritus.

The itching usually begins to be manifested in the third trimester, but may occur any time from before 20 weeks up until term.

It ranges in intensity from mild to virtually intolerable. It is usually noticed first at night and then progresses to become constant.

Though the pruritus is found throughout the body, it is often most severe on the palms and the soles.

Insomnia often accompanies the itching and the patient may scratch her skin to the point it becomes excoriated (scratched and crusted over).

Though the term "itchy" may conjure notions of mere annoyance, it should be understood that the pruritus associated with ICP has been described as "disabling", "severe and incapacitating", and on rare occasions, has even lead to threats of suicide.

After delivery, the symptoms usually decrease within 48 hours of delivery and disappear completely within 4 weeks postpartum.

Cholestasis of pregnancy does not cause permanent liver impairment once the baby is delivered, the liver returns to normal function.
