This disease has been referred to as the great impersonator because of the variety of symptoms observed during the initial manifestation and course of the disease. The clinical syndrome can present in 3 types of symptoms including: |
-
an acquired intracorpuscular hemolytic anemia due to the abnormal susceptibility of the red cell membrane to the hemolytic activity of complement;
-
thromboses in large vessels, such as hepatic, abdominal, cerebral, and subdermal veins; and
-
a deficiency in hematopoiesis that may be mild or severe, such as pancytopenia in aplastic anemia state. The triad of hemolytic anemia, pancytopenia, and thrombosis makes PNH a truly unique clinical syndrome.

PNH blood cells are deficient in an enzyme known as PIG-A, which is required for the biosynthesis of cellular anchors. Proteins that are partly on the outside of cells are often attached to the cell membrane by a glycosylphosphatidylinositol (GPI) anchor, and PIG-A is required for the synthesis of a key anchor component. If PIG-A is defective, surface proteins that protect the cell from destructive components in the blood (complement) are not anchored and therefore absent, so the blood cells are broken down. |
The PIG-A gene is found on the X chromosome. Although not an inherited disease, PNH is a genetic disorder, known as an acquired genetic disorder. The affected blood cell clone passes the altered PIG-A to all its descendantsred cells, leukocytes (including lymphocytes), and platelets. The proportion of abnormal red blood cells in the blood determines the severity of the disease. |
The result of the loss of these cell surface proteins is an enhanced sensitivity to complement-mediated cell destruction. (Complement is a substance produced by the immune system.) The disease can affect people of any age. |
Red blood cell, white blood cell, and platelet counts may be low. Urine may be intermittently red or brown, signifying the breakdown of red blood cells with release of hemoglobin into the circulation and then the urine. |
Blood clots may form in some people. The disease may arise in relation to aplastic anemia, and may progress to acute myelogenous leukemia. Risk factors, except for prior aplastic anemia, are not known. |
More importantly, PNH can cause aplastic anemia and thrombosis. Aplastic anemia may warrant blood transfusion (transfused blood is not hemolysed), and thrombosis can occur in the vein tree and less commonly in arteries. A very characteristic form of venous thrombosis is Budd-Chiari syndrome, thrombosis of the hepatic vein, and cerebral venous thrombosis. |
