DESCRIPTION Bronchiectasis is an abnormal destruction and dilation (widening) of the large airways. A person may be born with it (congenital bronchiectasis) or may acquire it later in life as a result of another disorder.

Bronchiectasis is often caused by recurrent inflammation or infection of the airways. It may be present at birth, but most often begins in childhood as a complication from infection or inhaling a foreign object.
Cystic fibrosis causes about 50% of all bronchiectasis in the United States today. Recurrent, severe lung infections (pneumonia, tuberculosis, fungal infections), abnormal lung defenses, and obstruction of the airway by a foreign body or tumor are some of the predisposing factors. It can also be caused by routinely breathing in food particles while eating.
Symptoms often develop gradually, and may occur months or years after the event that causes the bronchiectasis.
Underlying causes of bronchiectasis in children include cystic fibrosis, ciliary dyskinesia, immune deficiency, aspiration, bronchiolitis obliterans and allergic bronchopulmonary aspergillosis. Localized bronchiectasis may be caused by an aspirated foreign body or be the result of infection.
Apart from in allergic bronchopulmonary aspergillosis, in which proximal airways are affected, medium-sized airways are the site of bronchiectatic dilatation. Classification of bronchiectasis is into three types:
  • In cylindrical bronchiectasis, there is smooth dilatation of the affected bronchi, which do not taper normally towards the periphery, but there is no actual loss of bronchi. There may be plugging of bronchioles.
  • Varicose bronchiectasis shows irregularity and beading of the affected bronchi. There is destruction and loss of the bronchial lumen peripherally.
  • Saccular bronchiectasis is characterized by increasing dilatation of bronchi towards the periphery, with ballooning, and a loss of bronchial subdivisions characterise saccular bronchiectasis.
Bronchiectasis is one of the chronic obstructive pulmonary diseases (COPD) and it can be complicated byemphysema and bronchitis. The disease is commonly misdiagnosed as asthma or pneumonia.
Bronchiectasis can develop at any age, begins most often in childhood, but symptoms may not be apparent until much later. Bronchiectasis can occur as part of a birth defect, such asprimary ciliary dyskinesia or cystic fibrosis. About 50% of all cases of bronchiectasis in the U.S. result from cystic fibrosis. It can also develop after birth as a result of injury or other diseases, like tuberculosis, pneumonia and influenza.
