Nosebleeds are very common. The nose contains many tiny blood vessels that bleed easily. Air moving through the nose can dry and irritate the membranes lining the inside of the nose, forming crusts. These crusts bleed when irritated by rubbing, picking, or blowing the nose.
The lining of the nose is more likely to become dry and irritated from low humidity, allergies, colds, or sinusitis. Thus, nosebleeds occur more frequently in the winter when viruses are common and heated indoor air dries out the nostrils. A foreign object in the nose or direct impact to the nose can also cause a nosebleed.
If you have a deviated septum, you may be prone to frequent nosebleeds.
Most nosebleeds occur on the tip of the septum, the cartilage that separates the nasal chambers. The septum contains many fragile, easily damaged blood vessels. This form of nosebleed is usually easy to stop. Less commonly, nosebleeds may occur higher on the septum or deeper in the nose. These higher or deeper nosebleeds may be harder to control.
Occasionally, nosebleeds may indicate other disorders such as bleeding disorders or high blood pressure.
Hereditary hemorrhagic telangiectasia (also called HHT or Osler-Weber-Rendu syndrome) may be evidenced by nosebleeds. This is a disorder involving a blood vessel growth similar to a birthmark in the back of the nose.
Blood thinners such as Coumadin or aspirin may cause or worsen nosebleeds.
There are two types of nosebleeds: anterior and posterior. If the bleeding is near the front of the nose, it is an anterior nosebleed. A posterior nosebleed occurs in the back of the nose and is not always characterized by rapid bleeding but may be a slow, steady ooze. It may be more difficult to locate the site of the damage in a posterior nosebleed than in an anterior nosebleed. An anterior nosebleed is usually not as severe or serious as a posterior nosebleed.
The most common causes of nosebleed are:
  • Idiopathic (most common)
  • Traumatic/blunt - nose picking (epistaxis digitorum), low humidity, foreign body
  • Infection - upper respiratory, acute/chronic rhinitis, acute/chronic sinusitis
  • Vascular abnormalities - sclerotic vessels of age, hereditary hemorrhagic telangiectasia, arteriovenous malformation
  • Neoplasm (especially when unilateral)
  • Hypertension (usually in combination with another cause)
  • Coagulopathy - hereditary (e.g., hemophilia), therapeutic or adverse effect of drugs, blood dyscrasias, leukemias, thrombocytopenia or platelet dysfunction
  • Septal perforation
  • Septal deviation (one side is overexposed to dry air)
  • Bleeding originating in a sinus (fracture, tumor)
  • Endometriosis (nasal ectopic endometrium)

SYMPTOMS
Symptoms of anterior nosebleed include intermittent or constant bleeding out of the front of your nose. Blood can flow from one or both nostrils and can flow into the throat.
Symptoms of posterior nosebleed include bleeding that stops and starts, rapid bleeding from the back of the nose, or a slow, steady ooze. Sometimes the blood flows back into your throat. Especially with posterior nose bleeding, you can lose blood quickly.
