DESCRIPTION Emphysema is a lung disease that involves damage to the air sacs (alveoli). The air sacs are unable to completely deflate (hyperinflation) and are therefore unable to fill with fresh air to ensure adequate oxygen supply to the body. |

Cigarette smoking is the most common cause of emphysema. Tobacco smoke and other pollutants are thought to cause the release of chemicals from within the lungs that damage the walls of the air sacs. This damage becomes worse over time, affecting the exchange of oxygen and carbon dioxide in the lungs. |
A naturally occurring substance in the lungs called alpha-1 antitrypsin may protect against this damage. People with alpha-1 antitrypsin deficiency are at an increased risk for this disease. |
Chronic obstructive pulmonary disease (COPD) is a general term for a group of diseases that cause progressive damage to your lungs. These diseases include chronic bronchitis, asthma and emphysema. Of the more than 16 million Americans with COPD, nearly 3 million have symptomatic emphysema and millions more are in the early stages of the disease before signs and symptoms appear. |
Unlike asthma, which occurs when the muscles in your airways tighten, emphysema causes a loss of elasticity in the walls of the small air sacs in your lungs. Eventually, the walls stretch and break, creating larger, less efficient air sacs that aren't able to handle the normal exchange of oxygen and carbon dioxide. |
When emphysema is advanced, you must work so hard to expel air from your lungs that breathing can consume up to 20 percent of your resting energy. Unfortunately, because emphysema develops gradually over many years, you may not experience symptoms such as shortness of breath until irreversible damage has already occurred. Treatments focus on relieving symptoms and avoiding complications. |
There are two types of emphysema that are named by their location of disease. Patients generally have one or the other types of emphysema. Although less common, some patients can have both types of emphysema. |
-
Panlobular (or panacinar) Emphysema - This type of emphysema is characteristic of a weakening and inflammation of alveoli at the end of the bronchioles. When destruction is very severe, the affected acinus disappears and the lungs appear "spider web-like" in x-rays. (See a gross anatomy photograph.) A mild version of this type of emphysema occurs as aging progresses. In younger people, this panlobular emphysema is caused by the body's inability to produce sufficient amounts of alpha-1 antitrypsin.
-
Centrilobular (or centriacinar) Emphysema - This type of emphysema affects single alveoli entering directly into the walls of terminal and respiratory bronchioles. The alveoli are destroyed, causing the airway walls to enlarge and the alveoli to then disappear. Therefore, the alveoli at the end acinus remain. This type of emphysema is commonly associated with chronic bronchitis.
|
Emphysema and chronic Bronchitis are the most common forms of Chronic obstructive pulmonary disease (COPD). |
