There are two pleurae, one around each lung. The pleura is a two-ply membrane that both encloses the lung and lines the chest cavity. This two-layer protective wrapping fits closely around the lung, with, normally, no actual space between the inner and outer layers. The layers are joined at the edges, so that the pleura might be compared to a closed balloon, completely empty of air and wrapped tightly around the outside of each of the lungs. |
Only a thin lubricating layer of fluid is between the inner pleural lining and the outer one. Ordinarily, the smooth linings and lubricating fluid serve to allow the lung free movement within the chest for normal breathing. The almost non-existent space occupied by the lubricant can develop into what is called a pleural cavity if its fluid contents increase. |
Pleurisy may develop in the presence of lung inflammation (pneumonia, tuberculosis), rheumatic diseases, chest trauma, certain cancers, and asbestos-related disease. The main symptom is pain over the chest wall at the site of the inflammation. In some circumstances, the pain may be felt in the shoulder. |
The pain is increased by deep breathing, coughing, and chest movement. The normally smooth pleural surfaces -- now roughened by inflammation -- rub together with each breath, and may produce a rough, grating sound called a "friction rub". This can be heard with the stethoscope or an ear held against the patient's chest. |
Fluid often accumulates at the site of pleural inflammation. A localized collection of fluid separates the lung pleura from the chest wall pleura, causing the chest pain to disappear even though the illness may be worsening. |
Large accumulations of fluid compromise breathing and may cause coughing, shortness of breath with rapid breathing (tachypnea), cyanosis, and retractions. |
There are two kinds of pleurisy. A "primary" pleurisy is an inflammation arising in the pleural tissues themselves, from a germ that attacked them directly, or perhaps from an injury or growth. A "secondary" pleurisy is an added effect from some other chest disease – pneumonia, for instance – in which the germs reach the pleura as well as the lungs themselves, or tuberculosis, or lung abscess, or tumor of the lung; almost anything wrong in the chest. |
There are two types of pleurisy: dry and wet pleurisy. In dry pleurisy, the more common condition, the inflamed pleurae rub directly against each other. In wet pleurisy, fluid oozes from the inflamed tissue into the space between the lungs and the chest wall. This fluid may compress the lungs, making breathing difficult. Both types of pleurisy often occur as complications of respiratory tract infections, such as pneumonia, viral infections, and tuberculosis, and are more likely to develop in persons who are highly susceptible to such infections. They also can be caused by a tumor or an injury. Some cases are due to certain gastrointestinal tract diseases, particularly of the liver and pancreas, which can inflame the diaphragm (the large muscle separating the chest and abdominal cavities) and the portions of the pleurae that cover the diaphragm. |
