The vocal cords (or folds) are two small muscles located within the larynx (voice box) that are responsible for voice production. They are found at about the level of the Adam's apple (thyroid cartilage) in the neck. The vocal cords are very small (about 18mm in length for males and 11mm in length for females). |
Most of the time the vocal cords lie apart, forming a "V" shaped opening. During speech, the vocal cords come together and produce sound by vibrating. Movement of the lips and tongue change this sound to create individual speech sounds. |
Each vocal cord muscle is moved by a nerve called the recurrent laryngeal nerve (one on each side). If the nerve does not function properly, the attached vocal cord cannot move. This is vocal cord paralysis. This paralysis can result in either breathing or speech problems. Damage to one vocal cord causes a "breathy" voice, but rarely any breathing problems. Damage to both vocal cords causes severe noisy breathing (stridor), but the voice remains strong. |
Vocal cord paralysis can result from injury, tumors, or surgery in the neck and upper chest. Brain tumors and stroke can also affect the nerves. Infectious diseases that damage nerves--like whooping cough, tetanus and polio--can also cause vocal cord paralysis. Vocal cord paralysis can also appear as a congenital defect. If congenital, the most frequent cause is a brain defect, which can often be effectively treated. |
Someone who has vocal cord paralysis often has difficulty swallowing and coughing because food or liquids slip into the trachea and lungs. This happens because the paralyzed cord or cords remain open, leaving the airway passage and the lungs unprotected. |
Vocal cord paralysis may be caused by a variety of disorders or diseases. These disorders can either directly interfere with the mechanical movement of the vocal fold, or they may impact the recurrent laryngeal nerve, the nerve that stimulates the muscles of the vocal fold. Most often, vocal fold paralysis is caused by the latter. |
- Impairment of the Laryngeal Nerve

The laryngeal nerve may be stretched or cut during head, chest or neck surgery. A number of operations may leave a vocal cord partially or completely paralyzed, including cardiac surgery, thyroid surgery, carotid endarterectomy (for blocked vessels), or operations on neck vertebrae. A phenomenon called synkinesis can occur, in which the nerve endings become improperly connected or crossed. As a result, when the brain sends signals to open or close the vocal folds (during breathing, speech, or eating), the nerves send the stimulation elsewhere or improperly. Synkinesis may be a temporary part of recovery after injury to the recurrent laryngeal nerve, or it may be a more permanent problem.

Neurological disorders or injuries may impair nerve function and cause vocal cord paralysis. These disorders include stroke, multiple sclerosis, Parkinson's disease, and head trauma.

Tumors or growths may press on the laryngeal nerve anywhere along its path from the brain to the larynx.

Certain upper respiratory or viral infections can lead to vocal cord paralysis.
- Mechanical Impairment of the Vocal Folds

A joint (the cricoarytenoid joint) attached to pieces of cartilage allows the muscles to move the vocal folds relative to each other. In some cases, however, severe arthritis and other joint mobility problems may cause this joint to become inflamed and immoblized. Long periods of intubation can also lead to immobility of the cricoarytenoid joint and subsequent vocal cord paralysis.

Squamous cell cancer or other tumors beneath the lining of the larynx can impede the movement of the vocal fold.

The voice is always affected; at best it is breathy and weak. At worst, it is not there at all. In infants, the cry can be weak. Older children will suppress laughing and coughing because it is hard to do.

Swallowing may be hindered so that food ends up in the airway, causing violent coughing and often leading to pneumonia.

Breathing is obstructed on inspiration, producing a condition known as stridor. Closing the airway while breathing in produces creaking noises in the throat and changes the shape of the chest. The breast bone is drawn inward, much more visibly in the flexible chest of a small child.
