A pneumothorax is commonly known as a collapsed lung. Normally, the outer surface of the lung sits next to the inner surface of the chest wall. Thin membranes called pleura cover both surfaces. A pneumothorax occurs when air escapes from the lungs or leaks through the chest wall and enters the space between the two membranes (pleural cavity). As air builds up, it causes the nearby lung to collapse. |
Most cases of pneumothorax result from an injury to the lungs or chest wall, such as: |

There are several types of pneumothorax, which are classified by cause. |
Spontaneous pneumothorax is the occurrence of pneumothorax without a clear cause. Primary spontaneous pneumothorax occurs when there is no known underlying lung disease. It is thought to be caused by the rupture of a small, air-filled sac in the lung called a bleb or a bulla. The disease affects tall, thin men between 20 and 40 years old most frequently. Cigarette smoking and family history are contributing factors. |
Secondary spontaneous pneumothorax is a complication of underlying pulmonary (lung) disease (COPD, asthma, cystic fibrosis, tuberculosis, whooping cough). |
Traumatic pneumothorax results from a traumatic injury to the chest. The trauma may be penetrating (stab wound, gunshot) or blunt (blow from a motor vehicle accident). Traumatic pneumothorax may coexist with hemothorax. A hematocrit of > 50% in a bloody pleural fluid indicates hemothorax. |
Tension pneumothorax is caused when excessive pressure builds up around the lung, forcing it to collapse. The excessive pressure can also prevent the heart from pumping blood effectively, leading to shock. |
A pneumothorax is classified as open or closed. A closed pneumothorax is one which the source for the introduction of air into the intrapleural space has closed so that no more air can enter. An open pneumothorax is one which air moves into the intrapleural space as the chest wall of the patient moves in and out. This is often referred to as a "sucking" chest wound. |
