DESCRIPTION Atrophic rhinitis (AR) is characterised by squamous metaplasia followed by atrophy. The nose becomes filled with foul smelling crusts. The smell from these crusts may cause the child to be ostracized.

In the early stages there are small dry areas with crusting, either with viscid secretions or without secretions. In later stages the nasal passages become enlarged and have a foul smell, progressing to ozena. It is sometimes the result of trauma, and vascular damage by radiation therapy, environmental irritants, and disease has also been implicated.
The cause of atrophic rhinitis is unknown, although bacterial infection frequently plays a role. The mucous membrane changes from ciliated pseudostratified columnar epithelium to stratified squamous epithelium, and the lamina propria is reduced in amount and vascularity. Anosmia results, and epistaxis may be recurrent and severe. Atrophic rhinitis is differentiated from other forms of chronic rhinitis by the abnormal patency of the nasal cavities, caused by atrophy of the blood vessels and the seromucinous glands in the lamina propria.
Atrophic Rhinitis is an uncommon disorder in modern societies. Atrophic rhinitis is one of many terms used to describe a clinical entity. Other names have included dry rhinitis, rhinitis sicca, open-nose syndrome, or ozena.
The findings of atrophic rhinitis are easily recognizable. The first sign often is the smell of the patient. In some cases, this can be severe. Paradoxically, this likely will cause distress to everyone except the patient, due to the prevalent finding of anosmia. Patients may relate that others have informed them of the smell. Some patients have also been noted to have clinical depression at presentation due to the social implications of the disease. Another prominent finding is nasal crusting. The crusts are often extensive, filling the entire nasal cavity, and may be present even if the patient reports recent sinus irrigations. Removal of these crusts may induce bleeding. Once the crusts have been removed, several other features may be noted. The mucosa is generally atrophic, with elements of squamous metaplasia present. The volume of the nasal cavity may appear large, either due to absence of turbinate tissue, or lateralization of the lateral nasal walls. Purulent discharge and septal perforations are not uncommon.
Once the diagnosis of atrophic rhinitis is made, an etiology should be sought. The causes are generally separated into two categories: primary and secondary. Primary AR is the classic form of the disease, and is felt to arise de novo. In modern society, this is often a diagnosis of exclusion, and any precipitating factors such as sinus surgery, nasal trauma, or radiation are considered exclusions. The causative agent in almost all cases of primary AR is Klebsiella ozenae.
Secondary AR is the most common form of the disease in developed countries. The most common cause is sinus surgery, followed by radiation, trauma, and granulomatous or infectious diseases. Sinus surgery alone comprises almost 90% of secondary AR in some studies. Common procedures implicated include partial and total turbinectomies (80%), sinus surgery without turbinectomy (10%), and maxillectomy (6%). Granulomatous diseases which may result in AR include sarcoid, leprosy, and rhinoscleroma. Infectious causes include tuberculosis and syphilis. While infection was once the most common etiology of AR, it is now rather rare in developed nations, comprising only 1-2% of cases. Although infection may not be the causative agent in secondary AR, superinfection is uniformly present, and is the cause of the crusting, discharge, and foul odor. In these cases, K. ozenae comprises a small proportion of secondary infections. Other known causes are radiation therapy to the nose and sinuses (2-3%), and nasal trauma (1%).
