The kidneys are a vital part of the body. Their main job is to remove waste products from the blood, which are passed out of the body in urine. There are about a million tiny filters in each kidney called glomeruli. If these filtering units get inflamed (swollen) for some reason this is called glomerulonephritis. It means the kidneys are unable to work properly. Salt and excess fluid can build up, leading to complications such as high blood pressure and possibly kidney failure. |
Glomerulonephritis is caused by several different disease states, including the following: |
-
Systemic immune disease such as systemic lupus erythematosus (SLE, or lupus)
-
Other systemic diseases may include:
- Polyarteritis nodosa group - an inflammatory disease of the arteries.
- Wegener vasculitis - A progressive disease that leads to widespread inflammation of all of the organs in the body.
- Henoch-Schönlein purpura - A disease usually seen in children that is associated with purpura (small or large purple lesions on the skin and internally on the organs) and involves multiple organ systems.
-
Glomerulonephritis can also result from a gene on the X chromosome passed on from carrier mothers who have no features, or minimal features of the problem, to their sons, who are affected with the disorder in 50 percent of the cases.
-
In children, a common cause of glomerulonephritis is from a streptococcal infection, such as strep throat or upper respiratory infection. Glomerulonephritis usually occurs more than one week after an infection. This is often referred to as acute poststreptococcal glomerulonephritis, or APSGN.
|
There are two types of glomerulonephritis - acute and chronic. The acute form develops suddenly. You may get it after an infection in your throat or on your skin. Sometimes, you may get well on your own. Other times, your kidneys may stop working unless effective treatment is started quickly. The early signs of the acute disease are: |
- puffiness of your face in the morning
- blood in your urine (or brown urine)
- urinating less than usual
|
The chronic form may develop silently over several years. It often leads to complete failure of your kidneys. Signs and symptoms of kidney failure include: |
- lack of appetite
- nausea and vomiting
- fatigue
- difficulty sleeping
- dry and itchy skin
- muscle cramps, especially at night
|
Glomerulonephritis may be a temporary and reversible condition, or it may be progressive. Progressive glomerulonephritis may result in destruction of the kidney glomeruli and chronic renal failure and end stage renal disease. |
Damage to the glomeruli with subsequent impaired filtering causes blood and protein to be lost in the urine. Because symptoms develop gradually, the disorder may be discovered when there is an abnormal urinalysis during routine physical or examination for unrelated disorders. Glomerulonephritis can cause hypertension and may only be discovered as a cause of hypertension that is difficult to control. |
It may develop after survival of the acute phase of rapidly progressive glomerulonephritis. In about one-fourth of people with chronic glomerulonephritis, there is no prior history of kidney disease, and the disorder first appears as chronic renal failure. |
Glomerulonephritis may be grouped as primary and secondary. In primary GN, only the kidneys are affected. In secondary GN, the kidneys are damaged as part of a more generalized disease that can affect other parts of the body. |
