There are two main types of renal failure:

Acute renal failure (ARF) - This is characterised by sudden impairment of renal function marked by rapid, and steadily increasing azotemia (accumulation in the blood of toxic products normally excreted by the kidneys) with or without oliguria (decreased urine output).
Chronic renal failure (CRF) - This is characterised by a slow, insidious, and irreversible impairment of renal excretory and regulatory function.

Unlike acute renal failure with its sudden reversible failure of kidney function, chronic renal failure is slowly progressive. Chronic renal failure results in the accumulation of fluid and waste products in the body, causing azotemia and uremia. Azotemia is the buildup of nitrogen waste products in the blood. It may occur without symptoms. Uremia is the state of ill health resulting from renal failure. Most body systems are affected by chronic renal failure. Fluid retention and uremia can cause many complications.

The kidneys filter wastes and excrete fluid by using the bloodstream's own natural pressure. There are numerous potential causes of damage to the kidneys:

Decreased blood flow -- this may occur when there is extremely low blood pressure caused by trauma, complicated surgery, septic shock, hemorrhage, or burns; associated dehydration; or other severe or complicated illnesses.

Infections such as acute pyelonephritis or septicemia.
Urinary tract obstruction, such as a narrowing of the urinary tract (stricture), tumors, kidney stones, nephrocalcinosis or enlarged prostate with subsequent acute bilateral obstructive uropathy.
Severe acute nephritic syndrome.
Disorders of the blood, such as idiopathic thrombocytopenic purpura (ITP), transfusion reaction, or other hemolytic disorders, malignant hypertension and disorders resulting from childbirth, such as bleeding placenta abruptio or placenta previa can damage the kidneys.
Autoimmune disorders such as scleroderma can cause acute renal failure.
In young children, hemolytic uremic syndrome is an increasingly common cause of acute renal failure. A toxin-secreting bacterium, Escherichia coli, found in contaminated undercooked meats, has been implicated as the cause of hemolytic uremic syndrome.

SYMPTOMS The symptoms and signs of renal failure depend upon the type of failure.

In acute renal failure, the patient may have oliguria (reduced urine output) along with generalised swelling (oedema) of the body together with high blood pressure. Sometimes there may be blood in the urine.

In the early stages of chronic renal failure, the patient may merely have increasing fatigue and tiredness and symptoms like nocturia (increased frequency of urination at night). Vomiting, oedema and high blood pressure eventually develop. There is also loss of appetite.

Other symptoms depend upon the cause of the failure. For example, if there are kidney stones, then there may be excruciating loin pain or if there is severe hemorrhage or diarrhoea there may be signs of shock.

Complications in renal failure include pulmonary oedema (presence of fluid in the lungs), hypertensive crisis (severe sustained increase in blood pressure), acidosis, hyperkalemia (increased level of Potassium in the blood), and infection. If untreated, the last stage of renal failure is almost invariably fatal.
