Urinary incontinence becomes more common as people get older but it does not only affect older people. It is twice as common in women than men and affects many women who have had children.
When you need to pass urine your bladder tells your brain it is full. Even when the bladder is full, the majority of people can control the need to urinate until they get to a toilet. Then your brain tells your pelvic floor muscles to relax and your sphincter (the opening at the neck of your bladder) to open. Your bladder muscle tightens (contracts) and pushes the urine out.
If you have incontinence, either the muscles or the nerve supply are not working properly to control your bladder. The two main types of urinary incontinence are stress incontinence (a small amount of urine leaks out during physical activity) and urge incontinence (the bladder empties completely).
Infants and children are not considered incontinent, but merely untrained, up to the time of toilet training. Occasional accidents are not unusual in children up to age 6 years. Young (and sometimes teenage) girls may have slight leakage of urine when laughing. Nighttime urination in children is normal until the age of 5 or 6.
Urinary incontinence can be caused by many different medical problems, including weak pelvic muscles or diabetes. Causes of urinary incontinence include:
  • For women, thinning and drying of the skin in the vagina or urethra, especially after menopause
  • For men, enlarged prostate gland or prostate surgery
  • Weakened pelvic muscles
  • Certain medicines
  • Build-up of stool in the bowels
  • Not being able to move around
  • Urinary tract infection
  • Problems such as diabetes or high calcium levels
Urinary incontinence is generally divided into the following types according to the malfunction involved:
  • Stress Incontinence - The most common type (60%). Stress incontinence occurs when the pelvic floor muscles weaken, allowing the bladder to push against the urethra. Damaged urinary sphincters can also cause stress incontinence. Urine emission occurs when the abdominal muscles push down on the lowered bladder. A slight loss of urine is likely to accompany everyday activities and events, such as laughing, coughing, sneezing, exercising, or lifting things such as little grandchildren.
  • Urge Incontinence - Characterized by a strong desire to urinate, followed by involuntary contractions of the bladder. Because the bladder actually contracts, urine is released quickly, making it impossible for urge incontinence sufferers to predict when the problem will occur. Urge incontinence can be caused by infections, sphincter disorders or nervous system disorders that affect the bladder. Blockages caused by bladder stones can also responsible for urge incontinence.
  • Overflow Incontinence - The bladder fills, but doesn't signal this to those with overflow incontinence. Once full, the bladder overflows and leaks small amounts of urine on an almost continuous basis. Men with enlarged prostates are often at risk for overflow incontinence. This condition is rarely seen in women.
  • Environmental Incontinence - Does not indicate a physical problem with the bladder or the urinary tract. However, external restrictions such as physical disabilities or mobility issues may prevent the patient from reaching the toilet on time.
  • Mixed Incontinence - Means that the patient suffers from a combination of bladder control disorders. The most common combination of disorders is stress incontinence and urge incontinence.
  • Transient Incontinence - Refers to temporary episodes of incontinence. Causes of transient incontinence might include medication side effects, infections of the bladder or urinary tract, or even severe constipation.
  • Nocturnal Enuresis - The medical term for bedwetting, a bladder control problem that affects thousands of children yearly. Although most children who suffer from nocturnal enuresis grow out of the problem as their bodies mature, bedwetting can remain a problem for some children well into their teenage years.
  • Functional Incontinence - The patient has mental or physical disabilities that impair urination, although the urinary system itself is normal. Conditions that can lead to function incontinence include the following:
    • Severe physical disorders, such as in Parkinson's disease.
    • Mental diseases, including Alzheimer's disease and other forms of dementia. Mental confusion may prevent both recognition of the need to void and locating a bathroom.
    • Severe depression. In such cases, people may become incontinent because they are indifferent to self-control.
