Bromhidrosis
DESCRIPTION Bromhidrosis is a condition of abnormal or offensive body odor determined to a large extent by apocrine gland secretion, although other sources may play a role. Bromhidrosis may originate from metabolic disorders; ingestion of foods, drugs, or toxic materials; or contact with various xenobiotics. Certain bacterial infections and contaminations also release volatile-smelling compounds.

Bromhidrosis exhibits a male predominance, which probably is a reflection of the fact that apocrine glands are more active in men than in women.

Most axillary, or armpit, sweat is odorless, and comes from the eccrine glands. The small amount of apocrine secretion in the sweat, however, causes the problem.
Although fresh apocrine sweat is also odorless, it develops its characteristic smell when the sweat is broken down by the hair and skin cells in the armpit, and then eaten by bacteria.
It's actually the bacteria that make the smell, releasing a chemical called 3-methyl-2-hexenoic acid.

The axilla is really the only area from which the distinctive apocrine odor is produced. SYMPTOMS Bromhidrosis is a metabolic and functional disease that often is not associated with any anatomic disturbance. The only symptom is the secretion of foul-smelling perspiration. The skin's appearance is essentially normal except when associated with other unrelated conditions (eg, intertrigo, erythrasma).
The diagnosis of apocrine bromhidrosis, can be easily made if the examiner has normal olfaction. There is, though, one very important distinction that must be made between the true bromhidrosis patient and the bromhidrosiphobic individual: the latter being people who have a morbid dread of bodily odors and unreal images about what they smell, or in this case, what they think they smell. This sensory delusion can be an early warning sign of schizophrenia. Olfactory hallucinations may also be a sign of neurologic disease.
