Eczema is most common in infants, and at least half of those cases clear by age 36 months. In adults, it is generally a chronic or recurring condition.
Acute eczema has a sudden onset with intense symptoms that usually subside. It is recognised by redness and swelling, with an ill-defined border and small blisters. Chronic eczema refers to a long-term skin condition, which persistant symptoms that recur frequently.
In acute eczema the skin is typically red and inflamed with papules, vesicles, and blisters. In chronic eczema the skin may show the same features but be more scaly, pigmented and thickened. Two of the most common forms of eczema are atopic eczema and seborrhoeic dermatitis.
Many things cause eczema. The commonest is a general allergic over sensitivity (atopy). This sort of eczema is known as atopic eczema, and it is linked with asthma and hayfever. That is, these conditions often run together in a family.
The other possible causes include:
  • Infantile eczema which often affects young babies. This may lead to a patch below their chins, which gets wettest from dribbling, and may be associated with cradle cap.
  • Contact with substances which irritate the skin chemically. This is caused by direct contact between the skin and the substance, which might be such things as detergents, soaps, diesel or engine oils, strong chemicals, cleaners etc.
  • Contact with substances which the body has become allergic to. Commonly this involves nickel, rubbers etc. If a woman was sensitive to nickel in the past it would cause a reaction where the bra hooks and suspenders came near her skin, as these typically contained nickel. Plastics have helped to overcome this risk, but jewellery and watches are still a common cause. Suddenly people need to spend a bit more on their presents to you!
  • Varicose veins can lead to a form of eczema affecting the lower legs. This is known as varicose or gravitational eczema. As well as treatment of the skin, it is important to improve the blood circulation in the legs, and for most people this includes wearing support stockings and staying active on your feet. Your doctor will discuss the options.
Many substances have been identified as itch "triggers" in patients with eczema, and triggers are not the same for every person. Many times it is difficult to identify the exact trigger that causes a flare-up. For some, it seems that rough or coarse materials coming into contact with the skin causes itchiness. For others, feeling too hot and/or sweating will cause an outbreak. Other people find that certain soaps, detergents, disinfectants, contact with juices from fresh fruits and meats, dust mites, and animal saliva and danders may trigger itching. Upper respiratory infections (caused by viruses) may also be triggers. Stress can also sometimes aggravate an existing flare-up.
Eczema is often referred to as atopic dermatitis. However, atopic dermatitis is only one of the many types of eczema. The following describes the different types of eczema.
  • Atopic dermatitis is the most common form of chronic eczema. It tends to run in families who also suffer from hay fever (allergic rhinitus) and asthma.
  • Contact dermatitis is a type of skin inflammation. It's a temporary reaction to an irritant or allergen (a substance that starts off an allergic reaction), as occurs in poison ivy
  • Seborrhea is eczema of the scalp. In adults, mild seborrhea is known as dandruff. In babies, it tends to look more severe, and is called "cradle cap".
  • Nummular eczema normally affects older adults, and its cause is unknown. Round patches of scaly inflamed skin may appear anywhere on the body. Winter is the peak period for this condition, which tends to affect dry skin.
  • Varicose eczema (or gravitational eczema) affects the lower legs of older adults. It is basically a problem of poor blood circulation.
  • Lichen simplex chronicus is a chronic skin condition caused by a "scratch-itch cycle" where something irritates the skin (such as clothing), causing the person to scratch the area repeatedly. With frequent scratching, the skin area becomes thickened and leathery. It often appears in times of stress and anxiety. Neurodermatitis is another name for lichen simplex chronicus.
  • Dyshidrotic eczema is an irritation characterized by clear, deep blisters that itch and burn in the region of the skin of the palms of hands and soles of feet.
