Costochondritis causes pain in the lower rib area or upper breastbone. Some patients fear they are having a heart attack. The most severe pain is usually between the breast and the upper abdomen. The pain may be greater when in sitting or reclining positions. Stress may aggravate this condition. Generally the third or fourth ribs are affected. However, any of the seven costochondral junctions may be affected, and more often than not more than one site is involved. The inflammation can involve cartilage areas on both sides of the sternum, but usually is on one side only.
Most commonly the cause of costochondritis is classified as 'idiopathic,' or unknown. This means that there is no identifiable cause for the condition. This does not imply that idiopathic costochondritis is any less painful of a condition than if the cause can be identified. The most common known cause of costochondritis is traumatic injury. For example, a car accident where the driver's chest strikes the steering wheel can cause costochondritis by injuring the ribs and cartilage on the front of the chest. Viral infections, usually upper respiratory infections, have also been identified as a cause of costochondritis. The most frequently affected age group is young adults between 20 and 40 years old.
The risk of developing costochondritis increases with any physical activity that causes trauma or strain to the ribcage. It is a benign condition usually of short duration that resolves completely. Sometimes though it can recur or become persistent.
Tietze syndrome is often referred to as costochondritis, which is an inflammation involving the same area of the chest, but also includes swelling.

SYMPTOMS
The predominant symptom is a sharp pain in the affected area, most commonly involving the second or third ribs, which are often very tender to touch. (Ribs are counted down from the top and the first rib lies under the collar bone). The pain is usually related to movement, coughing and sneezing and can occur in more than one place simultaneously. Pain can also radiate into the arm and can be associated with a feeling of tightness in the chest. These are also the symptoms of a heart attack, so it is always necessary to take them seriously and make the diagnosis of costochondritis with care.
