Polymyositis (PM) and dermatomyositis are related diseases, both involving inflammation -- swelling or irritation -- of the voluntary muscles, those that normally govern movement, such as the muscles of the arms and legs.
Myositis means inflammation of muscle. Polymyositis means inflammation of many muscles, and dermatomyositis means inflammation of muscle and skin. The results of either condition are weakened muscles and often muscle pain. These conditions are seldom fatal, but in the most severe cases a person may need a wheelchair or require assistance with activities of daily living.
There are many different types of myositis, including:
  • Idiopathic inflammatory myopathies — In this group of muscle diseases, the cause of the muscle inflammation is unknown (idiopathic). There are three major types: dermatomyositis, polymyositis, and inclusion body myositis. All three illnesses cause muscle weakness, but the pattern of weakness varies. In the United States, idiopathic inflammatory myopathies affect about one out of every 100,000 people. Polymyositis and dermatomyositis are most common in women, whereas inclusion body myositis affects men more often. Symptoms can start at any age, but the average age of first symptoms is a bit younger in dermatomyositis and polymyositis (age 50) than in inclusion body myositis (age 60). So far, most of the evidence suggests that polymyositis and dermatomyositis are autoimmune disorders, illnesses in which the immune system mistakenly attacks the body's own cells. The muscles of people with inclusion body myositis contain an abnormal protein called amyloid, but the reason it forms is unknown. This amyloid protein is similar to the protein deposited in the brains of people with Alzheimer's disease, and some experts believe that the two diseases may develop the same way. In dermatomyositis, cancer is found in approximately 10% to 20% of cases. Sometimes, the muscle problem develops first. In other cases, the cancer is detected before the myositis.
  • Infectious myositis — Myositis sometimes occurs as part of a systemic (whole body) infection, especially a viral infection. It is especially common in people who have the flu (influenza). Myositis also can be caused by trichinosis, an infection in which tiny parasites invade the muscles. People can develop this infection by eating meat that has not been cooked enough. One type of infectious myositis is called pyomyositis, a bacterial infection that causes one or more pockets of pus (abscesses) inside a muscle. It usually is caused by Staphylococcus ("staph") bacteria. Pyomyositis is a relatively common infection in developing countries, where sanitation and health care are poor. However, it sometimes occurs in the United States, primarily in people who inject illegal drugs and in people infected with HIV.
  • Benign acute myositis — In benign acute myositis, a young child suddenly develops severe leg pain and cannot walk normally. These symptoms are dramatic and frightening, but they disappear within a few days. Benign acute myositis usually occurs in children who are recovering from the flu or some other respiratory infection caused by a virus. Doctors are not sure whether the child's muscle symptoms are caused by the virus itself or by the body's immune reaction to the virus.
  • Myositis ossificans — In myositis ossificans, a lump of bony material forms inside a muscle. This usually happens after a muscle injury, especially a bruise.
  • Drug-induced myositis — In drug-induced myositis, muscle inflammation occurs as a side effect of a medication or a combination of medicines. Although this is rare, the most common medications that can cause myositis are the cholesterol-lowering drugs called statins — including atorvastatin (Lipitor), lovastatin (Mevacor) and simvastatin (Zocor) — especially when combined with other similar drugs, and zidovudine (Retrovir), also called AZT, a drug used to treat HIV/AIDS.

SYMPTOMS
  • Muscle weakness and pain - Usually, the main symptom of polymyositis is muscle weakness. At the onset of the disease it generally affects the thighs and hips. If you have polymyositis, you may have difficulty climbing stairs, getting off buses with high steps or rising from low surfaces. The distance you are able to walk may eventually be limited because your muscles get too tired. If your shoulders are involved, you may have trouble getting dressed, combing your hair or lifting heavy objects. If polymyositis affects your neck, chest and abdomen muscles it may be hard to lift your head from a pillow or get out of bed. Polymyositis can also cause you to have trouble swallowing or talking, though these are less common results of the disease. At the start of the disease your muscles may feel sore and tender. Many people do not experience muscle pain; it occurs particularly in people with rapidly progressing disease. The muscle pain and tenderness usually goes away with treatment. After having the disease for a long period of time you may lose muscle bulk overall and the affected muscles may appear thin and wasted.
  • Skin rash - When a skin rash accompanies polymyositis, the term dermatomyositis is used to describe the disease. If you have dermatomyositis you will have muscle weakness and will develop a rash, most often on your face, scalp, neck and chest. The rash can also appear on your hands and fingers (often the knuckles), elbows, knees, ankles, upper arms and thighs. The rash is often deep red in colour (almost purple) and in some areas may be slightly raised.
  • Joint pain - Joint pain commonly occurs during periods when the disease is active, but the joints are not usually warm or swollen, as often happens with other forms of arthritis.
  • Lung involvement - Polymyositis and dermatomyositis can cause weakness of the muscles required for breathing. They may also cause fibrosis (build up of excessive fibrous tissue) of the lungs. If you have one of the diseases and your lungs are involved you may experience coughing and shortness of breath.
  • Heart involvement - In rare cases myocarditis (inflammation of the muscular walls of the heart) and congestive heart failure (heart disease accompanied by breathlessness and excessive retention of sodium and water) can occur as a result of polymyositis or dermatomyositis.
  • Calcium Deposits - Calcification (pronounced cal-si-fi-kay-shun) is hardening of skin and muscles as a result of calcium salt deposits. Calcification doesn't often occur in adults with the disease, but children with dermatomyositis may develop calcium deposits years after the disease starts. The deposits generally develop in the shoulder, pelvis, hip, calf and thigh and may severely limit motion. The masses that develop under the skin can rupture and the calcium salts may drain.
