Lumbar spine osteoarthritis
DESCRIPTION Osteoarthritis is a type of arthritis that is caused by the breakdown and eventual loss of the cartilage of one or more joints. Cartilage is a protein substance that serves as a "cushion" between the bones of the joints. Osteoarthritis is also known as degenerative arthritis. Among the over 100 different types of arthritis conditions, osteoarthritis is the most common. Osteoarthritis occurs more frequently as we age. Before age 45, osteoarthritis occurs more frequently in males. After age 55 years, it occurs more frequently in females.

Osteoarthritis commonly affects the hands, feet, spine, and large weight-bearing joints, such as the hips and knees. Most cases of osteoarthritis have no known cause and are referred to as primary osteoarthritis. When the cause of the osteoarthritis is known, the condition is referred to as secondary osteoarthritis.
Primary osteoarthritis is mostly related to aging. With aging, the water content of the cartilage increases and the protein makeup of cartilage degenerates. Repetitive use of the joints over the years irritates and inflames the cartilage, causing joint pain and swelling. Eventually, cartilage begins to degenerate by flaking or forming tiny crevasses. In advanced cases, there is a total loss of the cartilage cushion between the bones of the joints. Loss of cartilage cushion causes friction between the bones, leading to pain and limitation of joint mobility. Inflammation of the cartilage can also stimulate new bone outgrowths (spurs) to form around the joints. Osteoarthritis occasionally can be found in multiple members of the same family, implying an heredity (genetic) basis for this condition.
Spinal osteoarthritis is fairly common. Osteoarthritis can develop in the joints that connect the upper part of the spine as well as in the joints of the spine itself. The bony outgrowths, or osteophytes, can be detected by X ray but usually don't change a person's appearance. Many researchers believe that lifestyle habits, including prolonged sitting, cigarette smoking, consumption of alcohol, and weight gain, contribute to the onset of spinal osteoarthritis. However, for the most part the primary cause is not known.
Osteoarthritis of the spine occurs in the neck or lower back region. In this case there is also wearing of the discs between the spinal bones or vertebrae. This type of osteoarthritis is called Spondylosis or Spondylitis.
Stiffness and pain in the neck or in the lower back can result from osteoarthritis of the spine. Weakness or numbness of the arms or legs also can result. Some people feel better when they sleep on a firm mattress or sit using back support pillows. Others find it helps to use heat treatments or to follow an exercise program that strengthens the back and abdominal muscles. In severe cases, the doctor may suggest surgery to reduce pain and help restore function.
When osteoarthritis occurs in the lumbar spine, degeneration of the intervertebral disks (spondylosis), and narrowing of the spinal canal (spinal stenosis) result. This degeneration and narrowing can cause nerve root (radiculopathy) and spinal cord compression.

SYMPTOMS
Osteoarthritis frequently affects the spine. Back pain is the most common symptom. Usually, damaged joints in the spine cause only mild pain and stiffness. However, osteoarthritis in the neck or lower back can cause numbness, odd sensations, pain, and weakness in an arm or leg if the overgrowth of bone presses on nerves. Rarely, the blood vessels supplying the back of the brain are compressed, causing vision problems, a whirling sensation (vertigo), nausea, and vomiting. Sometimes bony growths compress the esophagus, making swallowing difficult.
