Low back pain means a pain or ache somewhere between the bottom of the ribs, at the back, and the top of the legs. The pain often begins suddenly, and may follow an obvious strain or injury, but may seem to come on "out of the blue", or come on slowly.
The pain may travel to or be felt elsewhere. It often goes into the buttocks, but may go further down the leg and even into the foot.
The pain may be worse on bending and is often worse sitting, especially in an easy chair. Sometimes turning over in bed and sitting up are agonising. Coughing or sneezing can often make the pain much worse. The muscles of the back may go into painful spasm.
Back pain may be combined with pain into the leg, travelling down below the knee, this is otherwise known as sciatica, because the main nerve to the leg (the sciatic nerve) is being irritated by pressure on it.
The common causes of low back pain are the following:
  • Muscle or ligament strain - Some of the largest, hardest-working muscles in the body are in or near the lower back. Unlike muscles stressed primarily during exercise, these muscles are also stressed with sitting, standing and walking.
  • Obesity - Increased weight on the spine and pressure on the discs often causes back pain.
  • Degenerative joint disease - The joints that allow mobility and stability are subject to wear and tear. In degenerative joint disease (also called osteoarthritis), the shiny, smooth cartilage that lines the joint wears away, leaving bone to rub on bone, a painful situation.
  • Disk disease - Given their location, torn or bulging disks can readily compress the spinal cord or nerves traveling out of the back.
  • Spinal stenosis - Narrowing (stenosis) of the spinal canal can be caused by calcium deposits in ligaments, degenerative joint disease or disk disease, or it may be present since birth. Any of these problems alone or in combination can lead to pressure on the spinal cord or a nearby nerve, causing pain.
  • Spondylolisthesis - Spondylolisthesis is a relatively common condition, especially among older persons, in which one vertebra slides forward on another. When a tiny defect or fracture in the bones at the back of the vertebrae is also present, the condition is called spondylolysis. In either case, the spinal cord or nerves leaving the cord can be compressed, causing pain in the back or legs.
  • Osteoporosis with compression fracture - By itself, osteoporosis is painless, but it increases the risk of fracture of the hip, wrist and vertebrae. The fracture itself or the resulting abnormal curvature of the spine or pressure on nerves may be a source of pain.
  • Tumors - Cancers and tumors of the spine and spinal cord are relatively rare. The most common symptom that patients with a spinal tumor have is pain.
  • Trauma - Trauma to the spine refers to injury that has occurred to bony elements, soft tissues and or neurological structures. The two things that surgeons are most concerned about, in the case of spinal trauma, are instability of the vertebral column and actual or potential neurological injury.
  • Excessive sexual activity - Low back pain is also caused due to excessive sexual activity and general weakness.
Low back pain may come on suddenly or gradually:
  • When low back pain is intense or "acute," it usually lasts a brief period such as a few days to several weeks.
  • Low back pain lasting longer than three months is termed "chronic."
  • Low back pain may or may not radiate down into the leg.
