DESCRIPTION Synovitis is the inflammation of a synovial (joint-lining) membrane, usually painful, particularly on motion, and characterized by swelling, due to effusion (fluid collection) in a synovial sac.

Synovial fluid is a transparent, viscid fluid secreted by the synovial membrane and found in joint cavities, bursae, and tendon sheaths.
Synovitis may be of numerous types, such as fibrinous, gonorrheal, tuberculous and rheumatic, among others.
Synovitis is a major problem in rheumatoid arthritis, in juvenile arthritis, in lupus, and in psoriatic arthritis. It may also be associated with rheumatic fever, tuberculosis, trauma, or gout.
Rheumatoid arthritis involves synovitis. In rheumatoid arthritis, the synovial membrane lining the joint becomes inflamed. The cells in the membrane divide and grow and inflammatory cells come into the joint from other parts of the body.
Synovitis can be acute (lasting for only a couple of weeks) or chronic (a persistent joint swelling that does not go away).
Non-specific synovitis is a term used to describe a secondary inflammation of the synovium not due to a specific cause such as bacterial (septic synovitis), immunogenic (rheumatoid synovitis) or pigmented villonodular synovitis.
Non-specific synovitis may develop with no apparent underlying cause but, most often, is secondary to a chemical, mechanical or a degenerative process. Common examples are the synovitis secondary to degenerative arthritis, bursitis secondary to repeated minor trauma, tenosynovitis secondary to overuse, subacromial bursitis secondary to degenerative supraspinatus tendinitis and synovitis secondary to crystal deposition (calcium pyrophosphates of pseudogout) or heavy-metal particles (lead from missiles).
There are two forms of proliferative disorder of the synovial membrane:
  • Villonodular synovitis - a proliferative disorder of the synovial membrane that is believed to arise in the synovial lining of joints, tendon sheaths, fascial planes or ligamentous tissue.
  • Pigmented villonodular synovitis - a proliferative disorder of the synovial membrane that most commonly involves the knee, hip, elbow and ankle. A brown fluid can frequently be aspirated from the joint, which reflects haemosiderin deposits from chronic haemorrhage or fresh blood from acute haemorrhage.
Toxic synovitis is a condition affecting children that causes hip pain and limping.

SYMPTOMS

Pain, swelling and erythema are the presenting symptoms and findings of acute synovitis while effusion and chronic discomfort and swelling are present in chronic synovitis.

Because of the mass of inflammatory cells in rheumatoid arthritis, the joint appears swollen and feels puffy or boggy to the touch. The increased blood flow that is a feature of the inflammation makes the joint warm. The cells release enzymes into the joint space which causes further pain and irritation. If the process continues for years, the enzymes may gradually digest the cartilage and bone of the joint leading to chronic pain and degenerative changes.
