Bell's palsy has been described as a diagnosis of exclusion because several other disorders present similar symptoms. Facial palsies have been linked to conditions such as Lyme disease, ear infection, meningitis, syphilis, German measles (rubella), mumps, chickenpox (varicella), and infection with Epstein-Barr virus (e.g., infectious mononucleosis). True Bell's palsy is an idiopathic facial palsy, meaning the root cause cannot be identified. Although Bell's palsy is not life-threatening, it can present symptoms similar to truly serious conditions, such as a stroke, ruptured aneurysm, or tumors.
In Bell's palsy, the muscles on one side of the face are weakened or paralyzed; palsy means paralysis, which is not being able to move. This makes it hard for the person to move one side of her face, and that side can end up looking stiff or droopy.
Bell's palsy was named after a Scottish doctor, Sir Charles Bell, who studied the nerve that directs how the face moves. This nerve, which is called the facial nerve, sends messages from the brain to the face. Through these messages, the facial nerve controls the muscles of your face and forehead. This means that any expressions you make - like raising your eyebrows, squeezing your eyes shut, smiling, or showing your teeth - are all controlled by the facial nerve. The facial nerve also helps your body make saliva (spit) and tears and helps you taste your favorite foods, like pizza or ice cream.
Each side of your face has its own facial nerve. The facial nerve starts in the brain and runs through a narrow tube of bone and out to the face from behind the ear. From there it splits into smaller branches of nerves that attach to the muscles of the face. Other small nerve branches run to the glands that make saliva, the glands that make tears, and the front of the tongue. Bell's palsy results from damage to the 7th (facial) cranial nerve.
When the facial nerve is irritated for any reason, it swells up and presses against the inside of the bony tube. This puts pressure on the nerve, and it can't send the right signals to the muscles in the face, the salivary glands, or the tongue, leading to Bell's palsy. Without those signals, half of a kid's face may seem to freeze. One eye may stay open, even if she's trying to close them both, and one corner of her mouth may look droopy or unhappy, even if she's trying to smile.
Bell's palsy can show up pretty fast. Someone who has it may go from being fine to having problems in just two days. Because the condition appears so suddenly, some people think they're having a stroke. But Bell's palsy is not a stroke and it is not dangerous. And even though it can be scary, Bell's palsy doesn't last forever. It usually goes away on its own within a few weeks but sometimes lasts as long as 3 months.
Bell's palsy can strike almost anyone at any age; however, it disproportionately attacks pregnant women and people who have diabetes, influenza, a cold, or some other upper respiratory ailment.
A specific cause of Bell's palsy is unknown, however, it has been suggested that the disorder may be inherited. It also may be associated with:
  • diabetes
  • high blood pressure
  • trauma
  • toxins
  • Lyme disease
  • Guillain-BarrÉ syndrome
  • sarcoidosis
  • myasthenia gravis
  • infection
