Cortisol is a glucocorticoid hormone, produced by the adrenal cortex. Aldosterone, a mineralocorticoid hormone that is also produced by the adrenal cortex, manages the salt and potassium balance in the blood. Adrenal insufficiency affects about 1-4 people per 100,000 in the U.S. It is found in patients of all ages and affects both males and females equally. Drug induced acceleration of hepatic metabolism of cortisol may also produce a relative adrenal insufficiency particularly in those with a limited adrenal reserve. Drugs which may produce this effect include phenytoin, barbiturates, and rifampicin. Drugs may also exert an inhibitory effect on cortisol synthesis e.g. ketoconazole and metyrapone. |
Secondary and tertiary adrenal insufficiency is due to problems relating to the pituitary gland and tertiary due to problems with the hypothalamus.
