DESCRIPTION Addison's disease (chronic adrenal insufficiency) is a disorder characterized by underactive adrenal glands and an insufficient production of the hormones cortisol and, sometimes, aldosterone. The adrenal glands are small organs located on top of each kidney. They consist of an inner layer called the medulla and an outer layer called the adrenal cortex. In the body, the hypothalamus, the pituitary glands, and the adrenal glands work together to produce hormones that control many body systems. If any part of their signaling and feedback system is not working it can cause major disruptions and illness within the body. |

Cortisol is a glucocorticoid hormone, produced by the adrenal cortex. Among its many roles, cortisol affects the metabolism of carbohydrates, proteins, and fats, affects glucose levels in the blood, acts as an anti-inflammatory agent, and helps the body react to stress. Aldosterone, a mineralocorticoid hormone that is also produced by the adrenal cortex, manages the salt and potassium balance in the blood. Without enough of these two hormones, the body becomes weak and dehydrated, unable to maintain an adequate blood pressure or to respond properly to stress. |
Adrenal insufficiency affects about 1-4 people per 100,000 in the U.S. It is found in patients of all ages and affects both males and females equally. Addison's disease is a term used as a synonym for primary adrenal insufficiency, caused by damage to the adrenal cortex; it does not present itself until 80 - 90% has been destroyed. The term "Addison's Disease" is often used incorrectly to refer to both primary and secondary adrenal insufficiency. |
Primary Adrenal insufficiency is due to the adrenal gland itself resulting in decreased levels of both cortisol and aldosterone , can be considered due to three causes. |
-
Due to a failure of the adrenal glands to form properly. Failure of the gland to form correctly would include such conditions as congenital adrenal hypoplasia and mutations of steroidogenic factor 1 (SF-1).
-
Due to destruction of the adrenal gland. Destruction of the gland may be due to autoimmune disease directed at the adrenal glands alone( 70%)or as part of a polyglandular syndrome, adrenoleukodystrophy, infections such as tuberculosis , or by metastatic disease, amyloidosis and trauma. Approximately 90% of the gland will have been destroyed before symptoms become evident
-
Due to a failure of the gland to produce steroids.1. Mitochondrial disorders, the Smith –Lemli-Opitz syndrome and defective cholesterol metabolism are some of the conditions which may produce impaired steroidogenesis.
|
Drug induced acceleration of hepatic metabolism of cortisol may also produce a relative adrenal insufficiency particularly in those with a limited adrenal reserve. Drugs which may produce this effect include phenytoin, barbiturates, and rifampicin. Drugs may also exert an inhibitory effect on cortisol synthesis e.g. ketoconazole and metyrapone. |
Secondary and tertiary adrenal insufficiency is due to problems relating to the pituitary gland and tertiary due to problems with the hypothalamus. Both pituitary and hypothalamus play a part in the regulation of cortisol production and therefore aldosterone deficiency is not a feature. Causes include: |
-
Long term glucocorticoid therapy (including inhaled steroids such as fluticasone) due to suppression of the hypothalamo-pituitary-adrenal axis. This effect can be seen for up to 12 months
-
Drug induced suppression of ACTH secretion e.g. sodium valproate
-
Pituitary or hypothalamic tumours, both benign and malignant
-
Metastatic tumours
-
Craniopharyngioma
-
TB
-
Post partum pituitary necrosis ( Sheehan’s syndrome)
-
Trauma
-
Post radiotherapy or surgery.
|
Addison's disease (primary adrenocortical insufficiency) is differentiated from secondary (pituitary failure) and tertiary (hypothalamic failure) causes of adrenocortical insufficiency. Mineral corticoid function usually remains intact in secondary and tertiary adrenocorticoid insufficiency. |
Addisonian (adrenal) crisis is an acute complication of adrenal insufficiency (circulatory collapse, dehydration, hypotension, nausea, vomiting, hypoglycemia); usually precipitated by acute physiologic stressor such as surgery, illness, exacerbation of co-morbid process, acute withdrawal of long term corticosteroid therapy |
