There are many different types of lipid particles (lipoproteins). Simple blood tests can determine levels of the most commonly measured lipoproteins, including total cholesterol, LDL and HDL cholesterol, and triglycerides. |
- Total cholesterol

An increasing total cholesterol level is associated with an increased risk of CHD. A "desirable" total cholesterol is usually <200 mg/dL. A total cholesterol of 200 to 239 mg/dL is considered borderline high, while a value 240 mg/dL is high. However, most decisions about treatment are made based upon the level of LDL or HDL cholesterol, rather than total cholesterol.

The low density lipoprotein (LDL) cholesterol (sometimes called "bad cholesterol") is a more accurate predictor of CHD than total cholesterol. Higher LDL cholesterol concentrations have been associated with an increased incidence of CHD in a large number of studies.

Ideally, LDL cholesterol levels should be less than 100 mg/dL in patients who have had CHD in the past or CHD risk equivalents. People with levels of 160 mg/dL or higher have a high risk of CHD. Intermediate levels — 130 to 159 mg/dL — predict an intermediate risk of CHD. CHD risk equivalents include noncoronary vascular disease (stroke, TIA, abdominal aortic aneurysm, lower extremity artery disease), diabetes, or any individual with a 10 year risk of CHD that is 20 percent or higher. People with multiple (2+) risk factors have a LDL cholesterol goal of less than 130 mg/dL, and those individuals with zero to one risk factors have a LDL cholesterol goal of less than 160 mg/dL.

Not all cholesterol is bad. Elevated levels of HDL cholesterol actually lower the risk of CHD. In fact, a very high HDL (60 mg/dL) is considered a "negative" risk factor for CHD by the United States National Cholesterol Education Program (NCEP) Guidelines [1]. That is, this level of HDL cholesterol removes one risk factor for CHD from the overall cardiac risk profile (see below).
