Almost all (95%) thyroid nodules or tumors are benign. They develop from thyroid follicular cells and can be found in normal-sized thyroid glands and goiters (enlarged thyroid glands). The thyroid may contain one nodule or several nodules. For example, multinodular goiter describes a gland with several benign nodules. Often these nodules are cysts filled with fluid. Or they may be "lumps" of stored thyroid hormone called colloid nodules. Colloid nodules have very few cells in them.
Although the vast majority of thyroid nodules are benign (noncancerous), a small proportion of thyroid nodules do contain thyroid cancer. Because of this possibility, the evaluation of a thyroid nodule is aimed at discovering a potential thyroid cancer.
Thyroid nodules are round or ovoid structures (lumps) that differ from the surrounding tissue of the thyroid gland. These nodules are very common: perhaps half of all people have at least one thyroid nodule, although most of them do not know it. Sometimes a person will detect a nodule himself, or a doctor will feel a nodule during a routine examination of the neck. In other cases, these nodules are discovered during tests for unrelated conditions.
Thyroid nodules can represent many different conditions. Reassuringly, about 95 percent of all thyroid nodules are caused by benign conditions, but a few are malignant (thyroid cancer). When a thyroid nodule is discovered, diagnostic tests can determine if the nodule is benign or malignant and can help guide treatment decisions. It is important to learn as much as you can about your thyroid condition and to take an active role in these decisions.
Most thyroid nodules do not cause any symptoms. Your doctor usually discovers them during a routine physical examination, or you might notice a lump in your neck while looking in a mirror. If the nodule is made up of thyroid cells that actively produce thyroid hormone without regard to the body’s need, a patient may complain of hyperthyroid symptoms. A few patients with thyroid nodules may complain of pain in the neck, jaw, or ear. If the nodule is large enough, it may cause difficulty swallowing or cause a “tickle in the throat” or shortness of breath if it is pressing on the windpipe. Rarely, hoarseness can be caused if the nodule irritates a nerve to the voice box.
We do not know what causes most noncancerous thyroid nodules to form. A patient with hypothyroidism may also have a thyroid nodule, particularly if the cause is the inflammation known as Hashimoto’s thyroiditis. Sometimes a lack of iodine in the diet can cause a thyroid gland to produce nodules. Some autonomous nodules have a genetic defect that causes them to grow.
The thyroid nodule is the most common endocrine problem in the United States. The chances are 1 in 10 that you or someone you know will develop a thyroid nodule. Although thyroid cancer is the most important cause of the thyroid nodule, fortunately it occurs in less than 10% of nodules. This means that about 9 of 10 nodules are benign (noncancerous). The most common types of noncancerous thyroid nodules are known as colloid nodules and follicular neoplasms. If a nodule produces thyroid hormone without regard to the body’s need, it is called an autonomous nodule, and it can occasionally lead to hyperthyroidism. If the nodule is filled with fluid or blood, it is called a thyroid cyst.
Common types of the benign thyroid nodules are adenomas (overgrowths of "normal" thyroid tissue), thyroid cysts, and Hashimoto's thyroiditis.
Uncommon types of benign thyroid nodules are due to subacute thyroiditis, painless thyroiditis, unilateral lobe agenesis, or Riedel's struma.

SYMPTOMS Most thyroid nodules don't cause signs or symptoms. Occasionally, however, some may become so large that you can feel or even see the swelling at the base of your neck, especially when you're shaving or putting on makeup. Men sometimes become aware of a nodule because their shirt collars suddenly feel too tight.
Some nodules produce too much thyroxine, a hormone secreted by your thyroid gland. The extra thyroxine can cause signs and symptoms such as sudden, unexplained weight loss, nervousness and a rapid or irregular heartbeat.
A thyroid nodule is more likely to be malignant if it:
  • Grows quickly or feels hard
  • Causes you to be hoarse or to have trouble swallowing or breathing
  • Causes enlarged lymph nodes under your jaw or in your neck
