The prostate is a gland of the male reproductive system. It is located in front of the rectum and just below the bladder, the organ that stores urine. The prostate is quite small - it weighs only about an ounce - and is nearly the same size and shape as a walnut. The prostate wraps around a tube called the urethra, which carries urine from the bladder out through the tip of the penis.
Prostatitis can appear in several forms. The acute form is the least common, but is also the most severe and may require hospitalization. Symptoms of chronic prostatitis tend to develop more slowly and aren't as severe as those of acute prostatitis.
Prostatitis may account for up to 25 percent of all office visits by young and middle-aged men for complaints involving the genital and urinary systems. The term prostatitis actually encompasses four disorders:
  • Acute bacterial prostatitis is the least common of the four types but also the easiest to diagnose and treat effectively. Men with this disease often have chills, fever, pain in the lower back and genital area, urinary frequency and urgency often at night, burning or painful urination, body aches, and a demonstrable infection of the urinary tract as evidenced by white blood cells and bacteria in the urine. The treatment is an appropriate antibiotic.
  • Chronic bacterial prostatitis, also relatively uncommon, is acute prostatitis associated with an underlying defect in the prostate, which becomes a focal point for bacterial persistence in the urinary tract. Effective treatment usually requires identifying and removing the defect and then treating the infection with antibiotics. However, antibiotics often do not cure this condition.
  • Chronic prostatitis/chronic pelvic pain syndrome is the most common but least understood form of prostatitis. It is found in men of any age, its symptoms go away and then return without warning, and it may be inflammatory or noninflammatory. In the inflammatory form, urine, semen, and other fluids from the prostate show no evidence of a known infecting organism but do contain the kinds of cells the body usually produces to fight infection. In the noninflammatory form, no evidence of inflammation, including infection-fighting cells, is present. Antibiotics will not help nonbacterial prostatitis.
  • Asymptomatic inflammatory prostatitis is the diagnosis when the patient does not complain of pain or discomfort but has infection-fighting cells in his semen. Doctors usually find this form of prostatitis when looking for causes of infertility or testing for prostate cancer.
Individuals with acute and chronic bacterial prostatitis have documented bacterial infections of the prostate. Patients with nonbacterial prostatitis have signs of prostatic inflammation but no signs of bacterial infection. Prostatodynia (or prostalgia) is characterized by complaints consistent with prostatitis but no signs of prostatic inflammation.
The term prostatodynia or chronic pelvic pain syndrome (CPPS) is used loosely to designate any unexplained complaints of chronic pelvic pain associated with either (1) nonspecific voiding symptoms and/or pain located anywhere in or around the groin, genitalia, or perineum or (2) the absence of pyuria and bacteriuria, with or without excess white cells or bacteria, on results from Gram stain and culture of expressed prostatic secretions (EPS) in male patients.
The bacteria that cause acute and chronic bacterial prostatitis get into the prostate from the urethra by backward flow of infected urine into the prostate ducts. Bacterial prostatitis is not contagious and is not considered to be a sexually transmitted disease.
Certain conditions or medical procedures increase the risk of contracting bacterial prostatitis. There is a higher risk if the man has recently had a catheter or other instrument inserted into his urethra, an abnormality of his urinary tract or a recent bladder infection.
The sexually transmitted disease gonorrhea may sometimes cause prostatitis, and tuberculosis may spread to the prostate. Parasites and fungi may infect the prostate gland. Some men whose prostatitis is not caused by any microorganism have microscopic collections of cells called granulomas in their prostate tissue. Whether viruses also may cause prostatitis is debatable.
Chronic non-bacterial prostatitis results in similar inflammation of the prostate gland, but it may not be due to the typical bacteria that cause urine infections. Organisms called chlamydia or mycoplasma may cause it. Some of these organisms may be transmitted by sexual contact. It may also be due to a chemical or immunologic reaction, perhaps even to urine itself flowing backwards into the prostate ducts.
There is no known cause for prostatodynia. It does not appear to be related to bacteria or inflammation of the prostate gland but may be related to a disorder of the nerves and muscles surrounding the prostate gland.

SYMPTOMS
Symptoms of acute bacterial prostatitis strike suddenly and can include:
  • Feeling unwell
  • Chills or fever
  • Low back pain
  • Aching round the thighs and genitals
  • Deep pain between the scrotum and anus
  • Pain and difficulty on passing water
  • Frequency of passing water
  • Pain on ejaculation
  • The prostate gland will also usually be hot, swollen and tender when the doctor examines it.
  • Sometimes, infection persists in the nooks and crannies of the prostate gland, despite treatment. When this happens, chronic prostatitis results.
Symptoms of chronic bacterial prostatitis vary, but may include:
  • Pain and discomfort in the prostate, scrotum, testes, rectum or tip of the penis
  • Aching in the lower back, lower abdomen or inner thighs
  • Watery discharge from the penis
  • Urinary problems such as urgency, getting up at night to pass water, pain on passing water
  • Pain on ejaculation
  • Premature ejaculation
  • Blood in the semen
  • Infection and swelling of the testes
The most common symptoms of chronic non-bacterial prostatitis are:
  • Pain/ache in the testicles, perineum, penis or rectum
  • Low backache, especially after intercourse
  • Burning on passing water
  • Urinary frequency
  • Decreased urinary stream
  • Discharge from the urethra, especially after sex
Symptoms of prostatodynia include:
  • Pain in the genital area
  • Pain with urination
  • Difficulty urinating that lasts for more than six months
