Orchitis
DESCRIPTION Orchitis is an inflammation of testicle tissue. It is usually a consequence of epididymitis, which is inflammation of the epididymis (the tube which connects the vas deferens and the testicle). Inflammation of the epididymis is much more common than orchitis.

Epididymitis and epididymo-orchitis are usually caused by a bacterial infection. Infection can result from surgery, the insertion of a catheter into the bladder, or the spread of infections from elsewhere in the urinary tract.
Acute orchitis may occur as a complication of epididymitis, or as a primary de novo inflammatory process. The latter is believed to be due to the haematogenous dissemination of systemic infection, such as mumps. Approximately 30 % of patients who have mumps will develop orchitis during the course of the illness. It is most common in post-pubertal boys (rare before 10 years of age). It usually manifests 4 to 6 days after the onset of mumps. In one-third of boys who get orchitis caused by mumps, testicular atrophy (shrinking of the testicles) will result.
Chronic orchitis may be due to tuberculosis, syphilis, or nonspecific granulomatous inflammatory change. The latter is relatively common, and while the aetiology is unknown, granulomatous orchitis is thought to be the result of an autoimmune response to spermatozoa. Chronic orchitis may present with mild testicular pain or swelling, and can be confused with testicular tumour both clinically and on imaging. Therefore, the correct diagnosis is often made only after orchiectomy.
A tuberculous orchitis, generally accompanied by an epididymitis, is always secondary to lesions elsewhere, especially in the lungs or bones. Tuberculous epididymo-orchitis is usually associated with renal tuberculosis.
Orchitis may also occur in conjunction with infections of the prostate or epididymis and, like those conditions, may occur as a manifestation of sexually-transmitted diseases such as gonorrhea or chlamydia. The incidence of sexually-transmitted orchitis or epididymitis is higher in men 19 to 35 years old.

SYMPTOMS Symptoms may include:
