The basic lesion is a combination of osteomyelitis and arthritis. Hematogenous spread or by direct extension of infection from a focus in adjacent bone. |
The initial involvement in the bone is usually the epiphysis or metaphysis. A round cystic lesion with ill-defined margins occurs and the lesion is filled with an inflammatory granulation tissue. Surrounding hyperaemia produces a wide area of osteopenia. The lesion is almost purely destructive or lytic, with little or no bone reaction.

Most skeletal tuberculosis affects the spine and it almost always begins in the vertebral body, usually the anterior one third. The most frequent site of involvement is the lower thoracic and upper lumbar spine (50% of patients); cervicalspine and lumber spine are each involved in 25% of patients. The disc is involved when 2 adjacent vertebral bodies are affected. Paravertebral abscess formation is characteristic and calcification may occur within the abscess. Severe kyphosis, sinus formation & (Pott's) paraplegia are all late sequelae. |
Skeletal tuberculosis outside the axial skeleton usually affects major joints, particularly hip and knee. It may present as gradually worsening arthritis but is often mistaken for some other form of arthritis such as mono-articular juvenile rheumatoid arthritis. Unlike most forms of arthritis, TB joint involvement is most often mono-articular. Effusion followed by synovial proliferation and thickening occurs in the joint Peri-articular osteopenia is common in TB arthropathy and overgrowth of the epiphyses may occur due to the hyperaemia. Subchondral erosion and joint space narrowing will follow. |
As the infection continues untreated, large amount of caseous material and pus accumulate and dissect along normal tissue planes. Eventually a sinus tract to the surface is formed forming a cold abscess because of the lack of any signs of acute inflammation. |
