DESCRIPTION Erysipelas is a superficial infection of the skin, which typically involves the lymphatic system. Erysipelas is also known as St. Anthony's Fire, an accurate description of the intensity of this rash. Erysipelas was a feared disease in pre-antibiotic days, especially in infants.

Erysipelas is most often caused by a specific Streptococcus bacteria known as Group A Streptococcus. In a few cases, it can be caused by other types of Streptococcus or Staphylococcus bacteria. Some cases of erysipelas have an inciting wound such as trauma, an abrasion, or some other break in the skin that precede the fiery infection. However, in most cases, no break in the skin can be found.
Erysipelas predominantly affects the skin of the lower limbs, but when it involves the face it can have a characteristic butterfly distribution on the cheeks and bridge of the nose. Symptoms and signs of erysipelas are usually abrupt in onset and often accompanied by general illness in the form of fevers, chills and shivering. Affected skin is distinguished from other forms of cellulitis by a well-defined, raised border. The affected skin is red, swollen and may be finely dimpled (like an orange skin). It may be blistered. Bleeding into the skin may cause purpura (purple patches). Cellulitis does not usually exhibit such marked swelling but shares other features with erysipelas such as pain and increased warmth of affected skin.
Erysipelas may be recurrent and may result in chronic lymphedema.
This highly unpleasant condition is most common in elderly people. It is also seen in the very young, and in people with immune-deficiency (due to HIV or cancer of the components of the blood for example).
A breach in the skin surface is a common route into the body for infectious organisms. Because of this, eczema, or even a small scratch, especially in the area around the ear or in the auditory passage can lead to erysipelas.
People with a leg ulcer or fungal infection on the foot are at increased risk, as these conditions weaken the normal defence mechanisms of the skin and make it easier for the bacteria to invade.

SYMPTOMS
  • An erysipelas skin lesion typically has a raised border that is sharply demarcated from normal skin. The underlying skin is painful, intensely red, hardened (indurated), swollen, and warm.
  • Facial erysipelas classically involve the cheeks and the bridge of the nose.
  • Blisters may develop over the skin lesion.
  • Fever and shaking chills are common.
