Schistosomiasis
DESCRIPTION Schistosomiasis is also known as bilharzia after Theodor Bilharz, who first identified the parasite in Egypt in 1851. Infection is widespread with a relatively low mortality rate, but a high morbidity rate, causing severe debilitating illness in millions of people. The disease is often associated with water resource development projects, such as dams and irrigation schemes, where the snail intermediate hosts of the parasite breed.

The main forms of human schistosomiasis are caused by five species of the flatworm, or blood flukes, known as schistosomes:
  • Schistosoma mansoni causes intestinal schistosomiasis and is prevalent in 52 countries and territories of Africa, Caribbean, the Eastern Mediterranean and South America
  • Schistosoma japonicum/Schistosoma mekongi cause intestinal schistosomiasis and are prevalent in 7 African countries and the Pacific region
  • Schistosoma intercalatum is found in ten African countries
  • Schistosoma haematobium causes urinary schistosomiasis and affects 54 countries in Africa and the Eastern Mediterranean.
Schistosoma infections are contracted through contact with contaminated water. The parasite in its infective stages is called a cercaria. It swims freely in open bodies of water.
On contact with humans, the parasite burrows into the skin, matures into another larval stage (schistosomula), then migrates to the lungs and liver (where it matures into the adult form).
The adult worm then migrates to the anatomic area of its preference, depending on which species is involved. Likely areas include the bladder, rectum, intestines, liver, portal venous system, spleen, and lungs.
Schistosomiasis is not usually found in the United States. However, it is prevalent in many tropical or subtropical areas, and it is a common illness thought to affect more than 200 million people.

SYMPTOMS
  • Symptoms vary with the species of worm and the phase of infection.
  • Initial invasion of the skin may cause itching and a rash (swimmer's itch).
  • Heavy infestation may cause fever, chills, lymph node enlargement, liver and spleen enlargement, and ascites.
  • Urinary symptoms may include frequency, painful urination (dysuria), and blood in urine (hematuria).
  • Intestinal symptoms include abdominal pain and diarrhea (which may be bloody).
  • A gross enlargement of the esophageal and gastric veins may result in their rupture. S. japonicum eggs are sometimes carried to the central nervous system and cause headache, disorientation, amnesia and coma.
  • Eggs carried to the heart produce arteriolitis and fibrosis resulting in enlargement and failure of the right ventricle.
