When people with TB in their lungs or throat cough, laugh, sneeze, sing, or even talk, the germs that cause TB may be spread into the air. It is important to understand that there is a difference between being infected with TB and having TB disease. Someone who is infected with TB has the TB germs, or bacteria, in their body. The body's defenses are protecting them from the germs and they are not sick. |
Tuberculosis of the breast is a rare disease. It is uncommon even in countries where the incidence of pulmonary and extrapulmonary tuberculosis is high. Not having well-defined clinical features, the true nature of the disease remains obscure and it is mistaken for carcinoma or pyogenic inflammatory disease of the breast. It also presents a diagnostic problem on mammography and ultrasound. The diagnostic criteria are the presence of granulomatous infiltrates and/or tubercles with central caseation, seen on histology or bacteriological culture of the aspirate. The disease is rarely bilateral and sometimes occurs along with breast cancer. |
Breast tuberculosis may be primary where the breast lesion is the only manifestation of the disease or it may be secondary in which a focus of tuberculosis has already been diagnosed elsewhere in the body and the disease appears in the breast at a later stage. The method of spread to the breast is by the hematogenous or lymphatic routes, or by direct extension from adjacent tissues. |
On a clinical and pathological basis, three different patterns of breast tuberculosis are described: a) nodular: well-defined, painless, slowly growing lesion, with late involvement of the overlying tissues, skin retraction, sinus formation and ulceration; b) disseminated: due to the confluence of multiple focal lesions; caseation, sinus formation and ulceration are described; c) sclerosing: occurs usually in older women. A slowly developing fibrosis is observed. Caseation is rare, while suppuration with sinus formation and ulceration can be observed. On a clinical and radiological basis, breast tuberculosis can mimic carcinoma, fibroadenoma, acute or chronic mastitis or actinomicosis. |
