Vogt-Koyanagi-Harada syndrome
DESCRIPTION Vogt-Koyanagi-Harada (VKH) syndrome, also known as uveomeningitic syndrome, is a systemic inflammatory syndrome that is characterized primarily by involvement of the eyes and meninges (coverings of the brain). The condition is most common in Asians and American Indians. Patients typically present with vision loss and headache, perhaps combined with but usually followed by vitiligo (patches of white skin), poliosis (patches of white hair, including eyelashes), alopecia (patchy hair loss), and hearing loss.

Ophthalmologists often find patients with VKH syndrome to have bilateral inflammation inside the eyes, in most cases this is manifest as uveitis (inflammatory changes of the iris, ciliary body, and choroid), which may result in bilateral serous retinal detachments with consequent abrupt and devastating visual loss.
The cause of VKH syndrome is unknown. The condition is thought to be an autoimmune process, in which the immune system mistakenly "attacks" one's own tissues. In this case, the skin, eyes, and meninges tend to be the sites of attack. It appears that pigmented cells in all of these anatomical sites are the final target of the immune system.
Unfortunately, there are no blood or X-ray tests that can tell an eye doctor without a doubt that a patient has VKH. Rather, VKH is diagnosed based on the history of the disease in a particular patient in combination with the findings present on examination and testing. The eye doctor will obtain a complete history and carefully examine the eyes for acuity or vision, for eye pressure, and for the presence of inflammation in the eye, using special instruments which magnify the eye under bright light. Special testing such as a fluorescein angiogram or ultrasound may be performed. Since no tests exist to diagnose VKH absolutely, the eye doctor will likely perform blood tests and a chest X-ray to ensure that other diseases which may look similar to VKH are not present. Some of these other eye diseases which can mimic VKH include sympathetic ophthalmia, sarcoidosis, intraocular lymphoma, uveal effusion syndrome, and multifocal central serous chorioretinopathy.

SYMPTOMS

The most noticeable symptom is a rapid loss of vision. Initial onset of Vogt-Koyanagi-Harada syndrome is characterized by severe headache, deep pain in the eye, vertigo (feeling that the room is spinning), and nausea, followed in a few weeks by uveitis and vision loss.

The other eye may become affected about 2 weeks later. The retina may detach, and hearing loss may become apparent. Facial nerve palsies (weakness) and rigidity as well as gait (walking) disturbance can occur.
