Vitreous hemorrhage
DESCRIPTION Vitreous hemorrhage (VH) is a condition of intraocular (inside the eye) bleeding that usually presents with sudden vision loss, frequently with complaints of floaters or "spots" in the vision. The condition is not due to bleeding of the vitreous humor, but rather bleeding from the retinal vessels or underlying choroid (layer of capillaries beneath the retina) in most cases. Occasionally, bleeding from the iris (in the front of the eye) results in the presence of blood in the vitreous humor.

There are a number of causes of vitreous hemorrhage. The most common is bleeding from abnormal blood vessels growing on the retina (neovascularization). When portions of the retina receive inadequate blood supply, usually as the result of diabetes or blockage in a blood vessel, these areas produce a chemical which stimulates blood vessel growth. Unfortunately, the blood vessels that grow in response to this chemical are abnormal - they grow from the retina into the vitreous gel and may bleed causing a vitreous hemorrhage. Other common causes of vitreous hemorrhage are retinal tears, and trauma.
When blood collects in the vitreous, it impairs the passage of light to the retina and results in foggy vision. Affected individuals often notice black strings in their vision followed later by a diffuse fog or haze. These symptoms often come on quite abruptly. The bleeding is usually short-lived but the blood may take anywhere from a few weeks to several years to be absorbed. Unfortunately, recurrent bleeding can occur and may result in a vitreous hemorrhage which persists indefinitely.
Hemorrhage density can vary significantly between patients who experience vitreous hemorrhage, but even a mild hemorrhage indicates the existence of a serious problem. Because of the absence of a validated and generally accepted medical definition of the various densities of vitreous hemorrhage, some people classify a vitreous hemorrhage as either mild, moderate or severe depending on the density of the vitreous hemorrhage as observed by the physician:
  • mild vitreous hemorrhage is characterized by trace blurring of retinal blood vessels
  • moderate vitreous hemorrhage is characterized by partial obscuration of retinal blood vessels and/or the optic nerve

Patients with acute vitreous hemorrhage frequently seek emergency care because the loss of vision is dramatic. Visual acuity varies with the degree of hemorrhage.
  • The patient recognizes minimal bleeding as new multiple floaters.
  • Moderate hemorrhage may be described graphically as 1 or more dark streaks that subsequently break up into numerous, minute black spots.
  • Dense hemorrhage can reduce vision to the light perception level.
Patients may report visual obstruction changes with eye or head movement and the tendency to try to look around the obstruction. In the absence of trauma, no pain is experienced with vitreous hemorrhage.
